You can be a guest (anonymous), click New post, and fill in any username you like.
Add a subject, and copy and paste this app into the body.
After you click send you'll be asked to type in some letters to verify you're a human.
My recommendation: After you have filled it out entirely copy and paste the whole thing, with your answers and store it into your clipboard.
If you make a mistake on the human verification part, you're returned to a blank message and have to start over.
